Chronic Care Management (CCM) is a Medicare program designed to provide ongoing support for patients with two or more chronic conditions. The goal is to help you better manage your health, reduce complications, and improve overall well-being through regular, personalized care coordination.
Are you eligible?
Patients are eligible for Chronic Care Management if they:
Have Medicare Coverage — check with your physician to see if you are covered.
AND
Have 2 Chronic Conditions that are expected to persist for at least 12 months.
*Common chronic conditions include chronic kidney disease, diabetes, heart disease, arthritis, hypertension, COPD, asthma and more.

How Does It Work?
Monthly Check-ins
A care coordinator from Toca Health will contact you each month to discuss your current health, medication management, and any challenges you may be facing. This personalized call is designed to monitor your progress and provide guidance.
Medication Management
Toca Health will assist with reviewing your medications, ensuring you're taking the right prescriptions at the right time.
Care Plan Development
Toca Health will create a personalized care plan based on your unique health needs, outlining the steps and services required to manage your conditions effectively.
Care Coordination
The Toca Health team will coordinate between your healthcare providers, specialists, and pharmacies, ensuring that all aspects of your care are aligned.
